This directive allows you to give advance directions about your future medical treatment in the event that you are unable to do so in the future.

The directive states your wishes or directions regarding your future health care for various medical conditions and comes into effect only if you are unable to make your own decisions.

You may have the directive apply at any time when you are unable to decide for yourself, or only if you are terminally ill.

Unlike an Enduring Power of Attorney, it relates to your health only and it is you that makes the decisions now rather than an Attorney making them for you in the future.
